Permission & Liability Release
Medical Treatment Authorization: In the event of illness or injury, I authorize Missio Church staff/volunteers to seek emergency medical treatment for my child, including hospital care, anesthesia, or surgery as deemed necessary by licensed medical personnel.
Travel Consent: I give permission for my child to travel with Missio Church staff/volunteers to Pennsylvania for this overnight trip.

Activity Participation: I give permission for my child to participate in all scheduled trip activities, unless otherwise noted.

Photo/Video Release (optional): I give permission for photos/videos of my child taken during the trip to be used in Missio Church communications (social media, website, print).

I understand that participation in this trip is voluntary and involves inherent risks, including travel, physical activity, and overnight lodging. I assume full responsibility for my child’s participation and hereby release and hold harmless Missio Church, its elders, staff, volunteers, and representatives from any and all claims of injury, illness, accident, or loss that may occur during or as a result of this trip, except in cases of gross negligence or intentional misconduct.

This release is binding upon me, my child, and our heirs, executors, and assigns.
Teen Snow Camp Interest Question
We are gauging interest for a possible overnight Winter Snow Camp at Camp Bayouca (Smithville Flats, NY) from January 16 (5pm) to Janary 17 (6pm). Cost would be $90 and includes meals from Friday dinner though Saturday dinner.

There would be Chapel, group games, snow tubing, crake stacking, and blacklight dodgeball.
