I am the parent or legal guardian of the student named above. I give permission for my child to participate in this Youth315 event sponsored by Missio Church.

I understand that participation in this event is voluntary and may involve ordinary risks, including but not limited to travel, walking, group activities, public venues, weather conditions, and other risks connected with participation in the event.

Medical Treatment Authorization

In the event of illness, injury, or medical emergency, I authorize Missio Church staff and approved volunteers to seek emergency medical care for my child when I cannot be reached in a timely manner. I understand that medical treatment will be provided by qualified medical professionals, and I accept responsibility for any medical expenses not covered by insurance.

Transportation Permission

I give permission for my child to travel with Missio Church staff or approved volunteers for this event, if transportation is being provided by the church.

Activity Participation

I give permission for my child to participate in all scheduled activities connected with this event, unless I note restrictions in writing.

Photo/Video Release

I give permission for photos or videos of my child taken during this event to be used in Missio Church communications, including church updates, social media, website, and print materials.

Release, Hold Harmless, and Indemnification

In consideration of Missio Church allowing my child to participate in this event, I agree, to the fullest extent permitted by law, to release, indemnify, defend, and hold harmless Missio Church, its elders, staff, Youth315 leaders, volunteers, and representatives from claims, costs, expenses, or damages arising out of my child’s participation in this event.

This agreement is intended to apply to claims brought by me or on my behalf, including claims for medical expenses, emotional distress, loss of services, or other damages connected with my child’s participation. This agreement is not intended to waive any rights that cannot legally be waived under New York law.

This release does not apply to claims arising from gross negligence, reckless conduct, or intentional misconduct.

By submitting this form, I affirm that I am the parent or legal guardian of the student named above, that I have read and understand this agreement, and that my electronic signature has the same effect as my handwritten signature.
