By submitting this form, I recognize and understand that I am participating in this EVENT (Soccer @ Skiddy) voluntarily and that the EVENT is optional, that I or my personal property may be exposed to some level of risk of injury or damage during the EVENT, and that I will be participating in all EVENT related activity at my own risk. I further recognize that I am responsible for my own actions. I further agree to accept any and all risks associated with any participation in this EVENT, including any serious injury or death to myself or damage to my personal property. 


Furthermore, in consideration of Missio Church allowing me to participate in this EVENT, I hereby do forever release from fault and hold harmless Risen Hope Community Church and its staff against loss (including court costs and reasonable attorneys' fees) from any and all claims of ordinary negligence, demands, rights, or causes of action of any kind or nature that may be brought by or on behalf of me arising out of any and all known or unknown, foreseen and unforeseen bodily or personal injuries, damages to property and consequences thereof which may be sustained by me arising out of or in connection with my participation in this EVENT, including but not limited to the travel to and from the EVENT if made available as part of the EVENT, except for such liability or claim of liability as may result from gross or intentional negligence on the part of Missio Church. I assume full responsibility for my participation in the EVENT and consent to release Risen Hope Community Church from liability. If I am under 18 years of age, THIS RELEASE AND WAIVER OF LIABILITY IS ALSO BINDING ON MY PARENTS' OR LEGAL GUARDIAN'S PERSONAL REPRESENTATIVES, HEIRS, AND ASSIGNS.
